HFCC-SSA Sick Leave Bank - Annual Enrollment

An extended sick and accident plan (Sick Leave Bank) has been established for all eligible HFCC-SSA employees, and is administered by a joint committee of three (3) HFCC-SSA members and three (3) members of the HFC Administration. This Sick Leave Bank is created for the benefit of employees who have exhausted their sick, vacation, and personal business leave as a result of a prolonged illness or injury. Click on the following links to see the authorization form and the guidelines:Sick Leave Bank Authorization FormSick Leave Bank RulesIf you are already a member of the sick bank, and wish to continue your membership, no further action is required on your part at this time; no additional days will be deducted from your personal sick bank this year.If you would like to become a new member of the Extended Sick and Accident Plan, or withdraw your membership, please return the attached authorization form to the Office of Human Resources no later than 4:00 p.m. on Wednesday, February 18, 2015.
