Initiation Fees and Union Dues forms

New Paperwork MUST be filled out and returned by January 30th.You will be receiving new paperwork for Initiation Fees and Dues in the mail. Wording of documents had to be changed and the old documents that were filled out have been destroyed.Fill out the Authorization for Check-Off of Dues form and return it to Barb Corp.If you still owe all or part of your initiation fees, you have two options, pay it in full now or have it deducted from your pay in increments of $6 per bi-weekly check until it is paid in full. If you want it automatically deducted from your pay, please fill out the Authorization for Initiation Fee document and sent it back to Barb Corp.
